Candidate Form - Identity details

The Identity Details section of the candidate form allows us to collect the information required to verify your identity.

1.      Type in your date of birth using the format provided. For example, 24.10.1985

2.      Select your gender from the drop-down list provided

3.      Type in any middle names that you have

4.      Select ‘yes’ or ‘no’ to the question that asks if you have been known by any other names

5.      Type in your national insurance number

6.      In the next box you need to type in your surname that was given to you when you were born. For example, your maiden name. This should only be filled out if your name is different to your current name

7.      Select your nationality from the drop-down list of options

8.      Enter the town in which you were born

9.      Lastly, from the dropdown list select the country you were born in
