Candidate Form - DBS (Disclosure and Barring Service) check

In this section of the candidate form you will be asked for some further details to allow us to conduct a DBS check.

1.      Enter the details of your current address using the boxes provided

2.      If you have a DBS profile number select ‘yes’ from the dropdown box and then type in the number. If you do not have a DBS profile number or you are not sure what this is select ‘no’. This is not a mandatory field

3.      Select ‘yes’ or ‘no’  under the question that asks if you would like a copy of the DBS certificate. If you select 'yes’ you will be asked if you want the certificate sent to your home address, if you want it sent to another address you will be prompted to type in an alternative address
